Vance is a Virtual Coverage Manager provided by PointCare that helps patients like you easily fill out and submit Medi-Cal enrollment forms. It also monitors coverage and alerts you when you need to re-enroll.
Vance offers several key benefits to simplify the Medi-Cal enrollment process:
- Coverage Loss Detection: Vance proactively monitors and detects Medi-Cal coverage losses, initiating immediate outreach via text to try and get you re-enrolled before your next visit, ensuring continuity of coverage and prescriptions.
- Virtual Re-Enrollment Anywhere: Vance submits Medicaid applications directly to Medi-Cal on your behalf, allowing you to re-enroll anywhere from your device.
- Bilingual Support: Vance communicates with you in both English and Spanish, removing language barriers and improving your enrollment experience.
No, Vance is completely free for you as a patient to use for Medi-Cal enrollment.

To complete your Medi-Cal application, you will need to provide the following information for you and your household members:
- Names
- Dates of Birth
- Address
- Social Security Numbers
- Tax filing information
- Any special health conditions or economic conditions
- Household Income information
And the following documents:
- Proof of Identity(Driver's License, Passport, Birth Certificate)
- Proof of Income (Pay Stub, Tax Return)
- Proof of Residence (Utility Bill, Rent/Mortgage Receipt)
- Proof of Citizenship Status (If non-citizen, I-94, USCIS documentation)
Vance is HIPAA-compliant so rest assured your personal and medical information is protected and remains confidential. Only HIPAA-compliant software and channels are used throughout the whole process and your data is stored in a secure cloud storage to protect your privacy.
Additionally, employees who will manage your application submission will be HIPAA-trained.
In order for Vance to submit an application on your behalf to Medicaid, you will need to sign an Authorized Representative form that is provided in the signature section of Vance. This will authorize PointCare to take actions on your behalf including:
- Complete and sign your application on your behalf
- Complete and sign renewal/redetermination forms

Medicaid Requirements and Information
Medi-Cal offers a range of benefits, including comprehensive medical coverage, prescription medications, doctor visits, hospital stays, and preventative care, all at little to no cost to you. Benefits may vary by state. Please see https://www.medicaid.gov/medicaid/benefits/index.html for more details on what is covered.
California allows undocumented immigrants to be covered by Medicaid. Please call the Vance Enrollment Services team at (925) 232-1925 and a team member will use an eligibility screening tool to verify your eligibility.
If the patient is deemed ineligible based on status, the patient may still qualify for the Medicaid emergency services category. This category will only cover services that are deemed emergent. For example, if you break your arm or are pregnant, this category of Medi-Cal will only cover medical services related to the broken arm or pregnancy. The Medi-Cal coverage will not extend to anything outside of that emergency service.
Provide as many details as possible in a written statement. The statement should be dated and include your income source should include as many details as possible including name, address, dates worked, supervisor name, and pay rate. The statement should be uploaded to Vance. Some states may require you to apply for unemployment benefits when no income is declared on the application.

Please list your address as homeless in the address line, then enter your City, State and Zip code. You will also need to enter a mailing address. If you don’t have a mailing address, we will use our mailing address and will contact you if we receive updates from Medicaid.

Types of Documents You Need to Submit Your Application
The following are examples of documents that can be accepted as valid proof of residence:
- Evidence the applicant has registered with a public or private employment agency in their state
- Current state-issued driver’s license or identification card
- Current and valid vehicle registration form in the applicant’s name
- Evidence the applicant is employed in their state
- Evidence that the applicant has enrolled his/her children in a state school
- Evidence that the applicant is receiving public assistance in their state
- Voter registration form of receipt, voter notification card, or an abstract of Voter of registration
- Current utility bill in the applicant’s name
- Current rent or mortgage receipt in the applicant’s name
The document should state your name or your dependant’s name, your annual income, and an issue date within the last 12 months.
The following are examples of documents that can be accepted as valid proof of income:
Earned Income - Non-Self-Employed Wage
- Pay stub
- Federal/State Income Tax Form 1040
- Wages and tax statement (W-2 and/or 1099)
- Employer statement
Earned Income - Self-Employment (includes farm income)
- Self-employment ledger documentation
- 1040 SE with Schedule C, F, or SE
- 1065 Schedule K1 with Schedule E
- Tax return
Unearned Income
- Unemployment benefits letter
- Statement of pension distribution from any government or private source
- Prizes, settlements, and awards, including alimony received and court-ordered award letter
- Social Security Administration Statements (Social Security Benefits Letter)
The following are examples of documents that can be accepted as valid proof of ID:
- State identification (ID) card
- Driver license
- US passport or passport card
- US military card (front and back)
- Military dependent’s ID card (front and back)
- Permanent Resident Card
- Certificate of Citizenship
- Certificate of Naturalization
- Employment Authorization Document
- Foreign passport
The following are examples of documents that can be accepted as valid proof of immigration status:
- Permanent Residence Card or “Green Card”
- Reentry Permit (I-327)
- Refugee Travel Document (I-571)
- Employment Authorization Document (I-766)
- Machine Readable Immigrant Visa (with temporary I-551 language)
- Arrival/Departure Record (I-94)
- Arrival/Departure Record (I-94) in foreign passport
- Foreign passport
- Certificate of Eligibility for Nonimmigrant Student Status (I-20)
- Certificate of Eligibility for Exchange Visitor Status (DS-2019)
What Happens Next
Once you submit your form, the Medicaid eligibility office will review your application and determine any additional information that is needed to make their decision.
It is very important to watch your mail for notifications from your local Medicaid office. The Vance Enrollment Services team is here to answer any questions about the notices you receive and walk through what is being requested from you. The Vance Enrollment Services team can assist you in submitting those verifications by uploading the documents to our portal (by request) if you are unable to take the documents to your local Medicaid office.
Please allow up to 45 days for the Medicaid office to make a decision once the application has been reviewed and any additional information, including income, household members, and employment details, has been provided. The decision will remain pending until all required information is submitted and evaluated.
